To invoke your policy benefits, complete this form or provide us with the following information relevant to your claim via email at or fax to 301-229-2482:

  1. Your full name and policy certificate number;
  2. Employing agency;
  3. Your contact information;
  4. Date of incident(s); and
  5. A brief description of the incident or matter related to your claim (i.e. subject of an investigation, disciplinary matter, RMO in an EEO case or civil suit).
