I am eligible for the up to ½ reimbursement of the professional liability benefit premium. How do I get the reimbursement from my agency?
Simply take your Certificate of Benefits to your Human Resources Office and ask them how you apply for the reimbursement of your professional liability insurance premium. Each agency handles reimbursement differently. Some agencies have you fill out a travel voucher for the reimbursement and others have developed a specific form for the reimbursement request.
